[MUSIC PLAYING] Welcome back. I'm Scott Fitzgerald. Fall means back to school for some full sports and for some parents getting their kids to wear those mouth guards can be a big challenge. Joining us is Dr. Pat Carroll with exceptional dentistry here in Kentucky. So Doc, here's the million dollar question. How do you get kids to wear these things when we see professional athletes? Not even wearing them. Great question, a couple of things. As you know, Scott, I've been in sports dentistry for a lot of my peers. I was a high school and competitive soccer referee for over 20 years. And so I've been around a lot. One is, at least 90% of the injuries that I have seen from sports related. They're not from football. They're not from hockey. They're not from field hockey. If there's one thing you can consistent with those three sports, at least in Kentucky, is you have to wear a mouth guard. If you're not wearing your mouth guard during the competitive part of the game, the referees will tell you, put an in or you're going out. Where I see my injuries in sports, if soccer, basketball, are probably the biggest two. And that's because they're not mandated. On my 41 years of dentistry, any dental injury I've ever treated, no one wearing a mouth guard have I ever treated from dental injury. To me, it's 100% effective. If you know where, you're probably gonna get two-thirds. If you watch the Olympics, Aja Wilson, arguably the best female basketball player in the country right now. That woman never takes her mouth guard out of her mouth. Watch that woman, go back, look at that. Aja has got her mouth guard in. And if the best female basketball player in the world can wear a mouth guard, we all can wear a mouth guard. You've gotta do that. As a parent, get 'em to wear it. You show examples like that. You bribe them, your friend. Yeah, whatever works for you to get your kid to do something, you can't tell you how important it is. Because Scott, if you lose a tooth as a youngster, we can't do an dental implant at a youngster. Your jaw bone growth has got to be done before we do it. So you're setting yourself up for 10 or 12 or however many years of wearing something removable to replace that, you're not gonna like it. Even if you do it in a limb plant at 20, you're gonna replace that crown a couple times over a year. You're setting your kid up for years of discomfort or inconvenience wearing something removable. And then, you know, paying some money to his dentist down the road until we learn how to grow teeth. And we're looking for that. That's an issue. So doc, for those not playing sports, but maybe need to replace that tooth. You think maybe implants might be the way to go. What say you? General implants have been the best thing that I have seen in my long career. When we were in dental school back in the dark ages, you know, we were either replacing teeth with something removable like goes in and out or what we call like fixed bridges where if you were gonna replace a missing tooth, you had to cut the tooth down on either side of it and cement something on each of those teeth with either one or two teeth in between it that were fixed. They could replace a tooth that a patient had lost. Don't be wrong, those are all great things. But like everything in healthcare, we keep advancing. And in the last 10 years, the restoration of people's teeth with dental implants has just been amazing. Let's just talk about someone who loses a single tooth. I myself have two dental implants. So I can speak from both professionally and personally what they're like. If someone loses a tooth now, we don't have to do anything to the teeth around it. We can give you something that is going to function pretty much just like your normal tooth used to do. What I talk to patients, one of the things there, the most certain sort of is actually the process. You know, they hear things about, I can get a tooth in a day, I can do this, I can do that. Yes and no, it's not usually that simple. So let's say you walked into me and we discovered a tooth that you had to have taken out. Talk about the alternatives and let's say you decided you wanted to do a dental implant. Well, there are two processes to the dental implant process. One, we use an oral surgeon. Now there are some general dentists that place their own dental implants, but we like to use an oral surgeon for that part. Of course, after talking to you and explaining the procedure, they remove the tooth as their, get that site ready for the dental implant, whether that's putting a little bone graft in or anything like that. And then not that same day, but after you've been for the consult, they will place the dental implant into your mouth. What I like to tell people to think about, that's kind of like a little root form piece of metal that the surgeon actually places up and through the gum and into the bone of your upper or lower jaw. Now, that sounds really, you know, if you say, ooh, that sounds bad. It's not. I've been there. It is a very non-painful procedure. But most of the time when you leave, they'll put the dental implant in there. Well, you usually have something kind of like a little manhole cover at the level of your gum. And if you can imagine, they place it by a little screw into the implant with a flat head that kind of lays level to the gum. That implant takes about three months to integrate. Used to be three, now some of the new implants, you've got like two months where that implant, when we say integrate, the bone actually grows to that implant. It gets solid in there. And during that time, depending on where the tooth is, we can make you something removable to replace that. If it's back in the back, you don't, most people don't do that. They just kind of go through that three months and let that healing process happen. - Again, joining us is Dr. Pat Carroll. He's with exceptional dentistry here in Kentucky. How important is the health of your jawbone when it comes to implants? - When people lose teeth the older way, then we were actually putting something in removable. The pressure on the bone and the tissue, you would actually see the bone resort as people get older. Now don't get me wrong, your jaw is not going to disappear. But you wouldn't see that bone get a lot smaller. When you place dental implants, we actually see that bone is preserved. The implant goes in there, the bone stays in there, so you don't see a change in the bone height. So that's a very good point. - And I think more importantly, too, it affects the other teeth in your mouth, right? You've got to do something to help you because those still kind of move around, don't they? - Correct. I tell people, think about your teeth as books on a bookcase, okay, in a bookshelf. And they're standing there all really pretty good. But if you take a book or two out of the middle of that bookshelf, those books are going to start leaning. Maybe you have two shelves and maybe the books on the bottom shelf or even helping support that top shelf a little bit. And if you take a couple books out of the middle of that lower one, you'll see that top shelf maybe start to sag a little. And that kind of happens, too. If you take out the bottom teeth and the top teeth have nothing to hit against, they'll sometimes start to kind of drift down. So it can really mess up the way your teeth come together if you don't replace those teeth. When you do that, the first part of the dental implant process is what the surgeon does. Then after that process of a couple months, the surgeon will say, okay, the bone's grown, everything's tight, let's get you back to your general dentist so they can place a tooth there for you. What we do at that point, you'll come to us and we'll unscrew that little screw for a black and better word. At this point now, we do digital scanning. I don't know if you've seen that yet. We'll place something in there, take a digital scan, which will then be able to communicate to our laboratory exactly the position of that implant, exactly how much tissue's there. And we just send that out to the cloud and to the laboratory. And then they fabricate for us a tooth that we can then screw into that dental implant. And it looks pretty much just like your tooth, man. It's an amazing process. It stays in there. It's not stuck together. You can floss between it. Say you lost four teeth. Well, we can put in a couple implants and then do like four teeth that we can kind of screw to those two implants. It can be used to replace one or four. The wonderful thing about dental implants is let's say somewhere down the road, a piece of that crown breaks that we screwed into your dental implant. Well, if that was a fixed bridge back in the old day, where it was two teeth connected together with something in the middle, you have to cut that old bridge off and do your whole new bridge. Well, in this case, we just unscrew that crown. We do a new scan and we make you a new crown. It's an easy repairable procedure. Much easier than we used to do. - So what can limit us from getting implants, Doc? - Say if you've had head and neck radiation for possible or localized cancer or something like that, that changes the quality of the blood vessels in the bone where the radiation went. And so you don't get a really good integration of the bone to the implant. So a lot of time patients that have had that kind of surgery or that kind of treatment are not good candidates. Some of the patients on what we call bisphosphonates now, which are medications for osteoporosis. There's some thought that that may lower the success rate of something like that. I have had very, very few dental implants fail. And most of them, if they're gonna fail, Scott, they'll fail pretty much right when the implant was put in, which means there was just something that wasn't exactly right at that point. And if that happens, they fail quickly. We can actually remedy that and do another implant. And that usually works well. Where I see implants fail down the road a little bit, the biggest thing, and I really look at people now and I make them sign extra stuff now, if you're a smoker. We know smoking is bad for a whole lot of things, whether it's the actual creating the suction and the nicotine constricts of blood vessels a little. You don't get a good blood flow. So I see some chronic failures. So smoking is really, really a big reason. Sometimes I just, I won't do them or we really have to have the patient sign a lot. Sometimes a patient does not have enough bone. You have to have enough bone or enough space between the teeth to place the dental implant. So it has a good place to grow. And so the surgeon can do some grafting and they can do some things. But sometimes there's nothing we can do. We can't modify that site enough to do that. And it's like anything in life these days. And plants are not an inexpensive procedure. Now, if you ask me, are they a great value? Yes, they're a great value. Give you an idea. We're talking like someone that walks into office who has no dental insurance. When we used to do what we call three in a bridge to replace one tooth, we would put a crown on either side and a false tooth in the middle. And that's about a $3,000 fee right now. A dental implant is actually gonna be a little bit more than that. You know, our fee in our office runs about $1,700 for our part. The surgeon's part is gonna probably run two to $3,000. It's hard to say depending on what they have to do. So instead of $3,000, you may be paying, say, $45, $4,800 if you had no insurance. Now, if you have to redo that bridge, but may last, the average life is seven to 10 years. Well, then it's another $3,000. And then if you, you know, if you're a 20 year, 30 year old guy and does it, you may replace that bridge two or three times in your life. So you're getting up to 10 grand or something. Whereas if you do a dental implant, you know, maybe you'll replace the crown down the road, but it's a lot better value. That being said, that's $3,000, $4,000, $5,000. That's a lot of money. Dental insurance, a lot of times is pain. There are portions on implants now. Most people do have implant coverage. But again, dental insurance is a whole nother topic. We can talk about another day how it really works. So they may pay half of that fee, maybe more depending on what's written. But then again, dental insurance has yearly maximums. Another topic for another day you and I should talk about. Again, we're spending a few minutes with Dr. Pat Carroll with the exceptional dentistry here in Kentucky. And for more information, just visit kintuckyannadental.com. Okay, Doc, a story came out recently regarding fluoride in our water and how it can lower the IQ in children. What'd you make of all this? If you're in a fluoridated area, you don't need to find a way for more systemic fluoride. If you go to your dentist, your high risk for cavities, you're someone that, "Hey, we're doing everything we can, but you know, "are you still getting a lot of cavities?" Then there are topical things we can do. Some patients we put on topical fluoride in the office or they're in a toothpaste that has prescription straight fluoride that again you brush on and you spit out. After reading this and talking to my family and stuff, I'm gonna say, "You know, gasses, "don't look for any more systemic fluoride. "You're in the area where you've got enough. "Listen to your dentist, your healthcare professional. "If they feel you need some more topical, "let's think about that and let's do it." I think point seven is about where I'm gonna, I'm not gonna just, you know, I'm not gonna recommend you try to find another source of ingestible or systemic fluoride. - Again, always great to catch up with you, Doc. On a new survey from Kelly Blue Book, found less than one in four consumers are confident in their understanding of financing options, basically when it comes to vehicle buying and the process that's involved in for those of us who aren't as sure while we turn to Brian Moody. He's the executive editor for Kelly Blue Book to help us out. Brian, thanks for taking time to join us. - Well, thank you for having me. - You bet. So Brian, let's start with the one area that matters most when it comes to car shopping and buying a new car, our own misconceptions. While we all think we know what we want and how to get it, what are most of us missing here, Brian? - Well, the number one thing to know is that if you're going to be borrowing money to buy a car and you don't have to do that, it would be better to pay cash if you can, but not everyone can do that. If you need to borrow money to buy a car, you need to know your credit score. That's important. In fact, in doing a survey, we found that 31% of respondents in the Southeast did not know their own credit score. And so that would include places like Louisville and other cities in Kentucky. Not knowing your credit score means that you don't know what kind of interest rate you're going to get or what deal you're going to get. When you see advertisements for 0% financing, 2.9% APR, all that stuff, that's based on credit score. So you need to know your credit score so that you can understand exactly what your total cost of buying a new or used car is going to be. - We're talking with Brian Moody, executive editor for Kelly Blue Book about buying that new car. And Brian, along those lines, is there a certain target score folks should be shooting for? Maybe if it's not as great, they can work on improving that. - Well, yes, of course. If you're in the seven and 800s, then that's considered premium or high. It's not about whether or not you have good credit or not. The point is that you need to know because you don't want to find out that your credit is moderate or poor when you're about to sign a document costing you thousands of dollars. So if you know ahead of time, you can make adjustments. You can say, I'm going to apply for a different loan or I'm going to get a less expensive car or I'm going to not get a new car right now and I'm going to work on my credit score first. But the point of this is just to say, you need to be educated about your own credit score because it determines a lot of things and can cost you money. - Sure, we're spending a few minutes with Brian Moody. In fact, getting an electric car might be a good example of how a leasing could work out well. You don't know if that's gonna be for you or not. You don't know where you're gonna charge it up or if the circumstance is gonna work for you. Leasing means at the end of that period, you're done. If it didn't work for you, you move on to the next thing. - Brian Moody's joining us executive editor for Kelly Blue Book. We're talking about the auto buying process and while you mentioned EVs, Brian, what are you all seeing in the EV market? Where do we go from here? What's that future looking like? - Well, a lot of people find that electric cars work well for them. I think if the average American were to drive an electric car, I think they'd like it. But that doesn't solve everything. Where to charge, how long it takes to charge. Those are big issues. And if you don't have a way to charge up at home, it might not be as convenient as you think. If you do have a way to charge up at home, it's gonna be the world's best experience. You're gonna love it. - Right. - So it really has to be the individual person assessing what are my needs, how is this gonna work for me? Electric car sales are continuing to grow, but they're growing at a slower rate than before. What I always tell people is look at a hybrid. A hybrid or a plug-in hybrid has the ability to run only on gas, and then it gets excellent fuel economy as well. - Spending a quality few minutes with Brian Moody. He is the executive editor for Kelly Blue Book. And Brian, one of the many things that is on the minds, I know of new car buyers. We've gone through this drill ourselves. Our interest rates, how are these going to affect car buying over the coming months? - We think interest rates are going to come down into late fall and into winter. And what that means is for a lot of people that will make credit more available, or they can afford a slightly more reliable car than they would have currently. Today, the interest rates are relatively high, unless you qualify for these pretty significant manufacturer incentives of low financing. So as the interest rates come down, there'll probably be more demand for new and used cars, and the prices may go up. If you're waiting for interest rates to come down before making your purchase, do all your research now, so that when the interest rate goes down, you can act quickly and not have to contend with rising prices as the year goes on. - Brian Moody's joining us, executive editor for Kelly Blue Book, talking about auto buying here, coming up in the fall months. And Brian, let me ask you, when it comes to purchasing a car, how to sort of buy yourself, cut out the middleman sites, change the game in the auto industry? - Well, there's more options than ever. That's the thing. There's consignment stores. There's big franchise dealerships. There's buying from online marketplaces. You can now go on sites like auto trader and others in search, no zip code, meaning I wanna see every Toyota Corolla that's for sale in the whole country. And that gives people more choices. It just makes that there's more options. The downside to some of these ways of purchasing a car is that you don't always know who's on the other end of the transaction. If you were to buy a used car from, say, I'm just making this up, from a Ford Honda or a Chevrolet dealer, it's a pretty good chance. They're not there to take advantage of you. They have a reputation to uphold and they wanna make sure that you're happy so that you tell all your friends, hey, what a great deal I had at blah, blah, blah Chevrolet. So you can get good deals. You can find lots of cars online and in these types of marketplaces you're talking about, but you really have to be cautious about the financial transaction if you aren't well aware of who's on the other side. - And last question for you again, we're talking with Brian Moody. He is the executive editor for Kelly Blue Book. Obviously, Kelly Blue Book is where a lot of us go to see our car's value. Brian, how important is the regular maintenance on your vehicles in maintaining that maintenance? 'Cause you know you don't wanna sell it down the road. - Right, it's important. It's also important to look for when you buy the car, is there a maintenance package that comes with the car? Maybe you pay it all upfront. Maybe you can get a discount. Those are things that you can sometimes negotiate and there are also many qualified shops outside of the dealership that can do the maintenance if you're looking to save money, but sometimes you don't save money because the requirements are the requirements. So it is important to do the maintenance on the car, especially if you're gonna keep it for a long time. And remember, the more you keep on the maintenance, the more it's gonna be worth because you can prove to the next buyer, look how well I took care of the car. - Glad you said that, Brian. I know I was gonna let you go, but I just had a major recall done on my car this past week. How important is it for folks to adhere to those recalls when they get those in the mail? - Very, in fact, what I always tell people is when you move to a new city, especially if it's a new state, immediately call or write to the dealership that's the selling brand of your car.
